How to Nominate

To nominate a charity, all you have to do is click on the link below and fill in the downloadable PDF form with as much information as possible.  Alternatively, please collect a form from the Lakeside Customer Information Desk on Level 2 by Brompton Walk.

Lakeside's Campaign Committee will then choose two charities every month from the nominations and reward them with a £500 contribution towards a specific project or piece of equipment that the charity requires.

Returning your Form

Email your completed nomination for to the Campaign Committee at lakeside-heart@capshop.co.uk.  Alternatively, print and return to the Lakeside Customer Information Desk on Level 2 or post it to: Julie Mellor, Heart of the Community, Lakeside Shopping Centre, West Thurrock, Grays, Essex RM20 2ZP.

Successful Applications: If your nominated charity is chosen, they will be contacted by the Campaign Committee via a letter informing them of their success and requesting more detailed information.

All other Applicants: Because of the volume of requests, we're unable to respond to every one, but would like to thank you for taking the time to send us information about your favourite charity.  Please don't hesitate to apply again in the future.
